High-ankle sprain (syndesmosis injury)

A high-ankle sprain is an injury to the ligaments that hold the two lower leg bones (tibia and fibula) together just above the ankle.

What is it?

A high-ankle sprain is an injury to the ligaments that hold the two lower leg bones (tibia and fibula) together just above the ankle. It is less common than an ordinary sprain but usually takes longer to recover.

What causes it?

  • The foot being forced to twist outwards, often in contact sports such as football and rugby.
  • It can happen together with an ankle fracture.

Common symptoms

  • Pain above the ankle, at the front, between the two leg bones.
  • Pain when twisting or pushing off.
  • Swelling and difficulty taking weight on the leg.

How is it diagnosed?

Your surgeon will use specific examination tests and X-rays taken while standing. An MRI or CT scan helps show whether the joint is stable.

Things you can do now

  • Protect the ankle and avoid twisting or pivoting until it has been assessed
  • Use ice, elevation and simple pain relief in the first few days
  • Follow a structured, gradual return to sport

Treatment

Treatment is tailored to your individual needs, symptoms and goals. Depending on your situation, it may include a protective boot with a period of protected weight bearing, physiotherapy and a structured return to sport and surgery to stabilise the joint if it is unstable.

Mr Jeyaseelan will talk you through the options that are right for you at your consultation.

When to see a specialist

  • Pain is above the ankle rather than on the outside, especially after a twisting injury
  • You can't walk normally after a sprain
  • You can't put weight on the foot after an injury, or it looks out of shape: go to A&E or an urgent treatment centre

What to expect

High-ankle sprains take longer to recover than ordinary sprains. Stable injuries heal in a boot, while unstable injuries are best fixed early to protect the joint.
