Your information

Privacy & cookies notice.

How we collect, use and protect your personal information when you use this website or contact us about an appointment.

Who we are

This website belongs to Mr Luckshmana (Lucky) Jeyaseelan, Consultant Trauma & Orthopaedic Foot & Ankle Surgeon (GMC 6164291). Appointment enquiries are handled by his practice team at Align Orthopaedics, 119 Harley Street, London W1G 6AU.

For the purposes of UK data protection law (the UK GDPR and the Data Protection Act 2018), the data controller for information you send us is Align Orthopaedics.

If you have a question about your information, contact us at hello@alignorthopaedics.co.uk or on 020 8058 2151, or write to the address above.

What information we collect

When you ask for an appointment

The booking form asks for:

  • your name, date of birth, email address and phone number
  • your preferred clinic and availability
  • whether you are self-paying or insured, and your insurer
  • a short description of your problem, and whether you have a referral letter

A description of your symptoms is health information, which the law treats as a special category of personal data that needs extra protection.

When you call or email us

We keep the details you give us so we can deal with your enquiry.

When you become a patient

If you go on to have a consultation or treatment, we create and keep medical records about your care. Each hospital where you are seen also keeps its own records and has its own privacy notice.

How the booking form works

When you press send, your request is sent over an encrypted (HTTPS) connection to our website host, Netlify, which passes it to our practice inbox by email. Netlify keeps a copy of each submission in our secure account so that requests aren't lost, and we delete these copies regularly. Netlify processes this information on our behalf under a data processing agreement and may store it outside the UK, protected by the safeguards UK data protection law requires.

If you'd rather not send health information online, leave the "reason" box brief or call us on 020 8058 2151.
