Flexor hallucis longus tendinopathy

The flexor hallucis longus (FHL) tendon runs behind the inner ankle to the big toe and helps you push off.

What is it?

The flexor hallucis longus (FHL) tendon runs behind the inner ankle to the big toe and helps you push off. In dancers and runners it can become irritated and sometimes catch, causing pain and clicking. It is sometimes called 'dancer's tendinitis'.

What causes it?

  • Repeated pointing of the foot and pushing off, especially in ballet.
  • Overuse.

Common symptoms

  • Pain at the back or inner side of the ankle.
  • Clicking, catching or stiffness of the big toe.

How is it diagnosed?

Diagnosis is by examination, with an ultrasound or MRI scan.

Things you can do now

  • Reduce the activities that bring on pain, such as rising onto the toes
  • Look at technique with your teacher or coach
  • Keep up general fitness

Treatment

Treatment is tailored to your individual needs, symptoms and goals. Depending on your situation, it may include rest, physiotherapy, changes to technique, a guided injection in selected cases and surgical release, often using keyhole techniques.

Mr Jeyaseelan will talk you through the options that are right for you at your consultation.

When to see a specialist

  • Pain at the back or inner side of the ankle persists
  • The big toe catches or triggers
  • It is affecting dance or sport

What to expect

Rest, physiotherapy and technique changes help most people. Surgical release, often by keyhole surgery, is effective when symptoms persist.
